Is this your very first PSU experience?

You must fill out a Quick Entry form, which gives us your address, etc. and puts you in the Student Information System (SIS). You can obtain the form by:

• emailing the SYSE Director, Herman Migliore

• download from PSU Quick Entry Form site

• going to the PSU admissions web site

• log-on and fill out online form

If you'll send me the copy of the Quick Entry Form, the program will pay the $10.00 fee, and I'll track progress:

1. Download the Quick Entry Form, fill it out;

2. Send to Herman Migliore by FAX 503.725.2825
   or postal mail:

3. After this form is processed within a few day, you can access the student data base. On-line registration involves entering the student database (SIS), which is password protected with a User ID and PIN. PSU will assign you a number that will be mailed to you. Your PIN for initial access will be your six-digit birth date, which should be changed for security purposes.  You pay the amount shown in your SIS account   Please be aware that there are due dates for payment; late fees and interest apply.
